6. By signing this Certification, I represent: I am the parent/legal guardian of the child or children listed below. I authorize my child to participate in the City of Lake Saint Louis Youth Baseball League (New Horizons League). I understand and acknowledge the League forbids children who 1) test positive for COVID-19 or 2) exhibit symptoms consistent with COVID-19 as outlined by the CDC and the Missouri Department of Health from participating in the League. I certify that in the event my child is diagnosed or experiences symptoms as described in sections 1 or 2 outlined above, I will not knowingly allow my child to play in the League. I also agree to notify the League and cooperate to the extent necessary to ensure the safety of others.
